Terms and Conditions

The following plan terms and conditions must be reviewed before enrolling in VIP’s Individual Vision Care Policy.

Rules & Regulations

ONLY VISION BENEFITS ARE PROVIDED BY THIS POLICY.

Monthly Installment Option:

If you decided to pay your premiums in monthly installments during the course of the policy’s term of benefits, you committed to do so:

  • If the policy has a 12-month term, you agree to pay the needed premium in twelve (12) equal payments. The initial amount will be deducted from your credit card or checking account on the day of enrollment, and the next eleven (11) payments will be deducted monthly on or around the auto-payment date you chose. VIP may revoke your coverage if a payment is not made for any reason. 30 days following the day on which your premium was due. By logging into your account on Visionplan.vip, you are in charge of updating your payment details.
  • If the policy is for a 24-month period, you agree to pay the required premium in 24 equal payments. The first installment will be deducted from your credit card or checking account on the day of enrollment, and the following 23 payments will be deducted monthly on or around the auto-payment date you chose. Thirty (30) days after the date your premium was due, VIP may cancel your coverage if a payment is not received for any reason. By logging into your account on visionplan.ViP, you are in charge of updating your payment details.

Billing for monthly installment payments occurs one month in advance. If you select the same month effective date, your initial payment will concurrently cover the current and following month’s installment periods. The following month, on or about the recurring auto-payment date you choose, your next payment will be deducted.

Renewal:

This Policy is renewable at the Policyholder’s choosing and will renew automatically if premiums are paid on time, the Policyholder hasn’t committed a fraud-related act or practice, and VIP continues to provide this plan. Due to a Covered Person’s medical needs for vision care services, VIP will not terminate coverage under the Policy. In the sixty (60) days previous to your auto-renewal, we will issue you a renewal letter.

This policy is not canceled if the renewal letter is not received. After thirty (30) days from the day your premium was due, VIP may cancel your plan if payment is not received for any reason.

Right to Return the Policy:

If, upon study of the Policy, you are not pleased with it for any reason, you may return the Policy within thirty (30) days after its delivery to you and get a refund of the premium paid, less the processing charge. The Policy will be null and void if you return it to VIP at its headquarters. This means that from the date the Policy became effective until the day the Policy was voided, you will be liable for full payment of any services obtained or materials bought. VIP Member Services must receive a formal cancellation request at visionplan.vip@gmail.com.

Other Insurance Coverage:

Any other private or public insurance plan, including any other plan VIP has underwritten, cannot be used to combine plan benefits payable under this Policy.

Grace Period:

of thirty-one (31) days will be given for the payment of each premium that becomes due after the first premium, provided that VIP has not delivered to the policyholder or mailed written notice of its intention not to renew this policy beyond the period for which the premium has been accepted, not less than thirty (30) days prior to the premium due date.

Limitations, Exclusions, and Exceptions:

Some eyeglass frame and lens brands might not be available for purchase as Plan Benefits or might be subject to additional restrictions. The Customer Care Division of VIP can be reached at 754-444-7177. Covered Persons can also speak with their VIP Preferred Provider to learn more about the brands of frames that are available. The eye exam and/or the purchase of the majority of materials are subject to copayments and other out-of-pocket expenditures. Cosmetic-related products or services are not covered by this policy. This coverage does not cover medical services or supplies. If any person insured by this policy visits a physician who is not in VIP’s provider network, their out-of-pocket costs will be higher.

You can choose the doctor who’s right for you. If you see an out-of-network provider, your benefit will differ and you’ll be required to submit a claim for reimbursement. Once your benefits are effective, login to vsp.com

Note: If you choose to see an out-of-network provider, you’ll receive less coverage. Payment is expected at the time of your visit. Following your appointment, submit your itemized claim to Vision Service Plan, Attention: Claim Services, P.O. Box 385018, Birmingham, AL 35238-5018. Out of network coverage is not available in the state of WA, MA, and varies in MD.

Your frame allowance indicates the portion of your frame cost that is covered by your plan. If your frame cost exceeds your plan’s allowance, you’ll also get 20% off any overage amount.

Should you choose to get contacts instead of glasses, if your plan allows, your allowance is applied toward contact lenses and/or your fitting & evaluation (required in addition to eye exam). Plus, you get 15% savings on your fitting & evaluation cost.

With the EasyOptions plan, you choose one of three upgrade options (see listed Benefit Options above). If you choose progressive (no-line multifocal) as your upgrade, they will be fully covered with $0 copay. If you choose a different upgrade option then your max cost to add no-line multifocals will be $175.
